Is your dog lonely while you
are at work? Does he destroy furniture, urinate in the house, or
dig up your garden? Does she get anxious when you leave her
alone?
Dogs are social animals and
love to be around other dogs and people. A bored dog can become
anxious and demanding, and may engage in destructive or
unhealthy behaviors. We’ll take great care of your best friend
while you are out for the day. Maybe your dog wants to play with
other dogs in our fun-filled pack environment. Maybe she wants
the special attention our experienced and loving staff provides.
Maybe he just wants to relax with his new friends in our Doggy
Lounge. Keep your dog happy by bringing him to The Muttley Crew!
In our fun and safe environment, your dog can play to her
heart’s content, get great exercise and socialization, and
release her pent-up energy. At the end of a fun-filled day, you
take home a tired but happy canine companion.
There are three required forms which need to be filled out
when bringing your pet to The Muttley Crew. For your convenience you may download
these forms off our website and fill them out prior to your arrival.

Daycare Rules and Regulations |
|
The purpose of The Muttley Crew is to provide a safe, fun
and stimulating environment for your pet. To ensure the safety and health
of your pet and of our other guests, we require all of our guests to
comply with the following Rules and Regulations.
AGE: All dogs must be 7 months of age or older.
SEX: All dogs must be spayed or neutered.
SHOTS: All dogs must have up-to-date vaccinations.
Owners must submit written proof that their pet received DHLPP, Rabies,
and Kennel Cough vaccination within the past 7 days to 11 months.
Specifically the vaccines required are Distemper, Hepatitis, Leptospirosis,
Parainfluenza, Parvovirus, and Bordatella. These shots are more commonly
referred to as DHLPP; 5 in 1 or 6 in 1 could include or have the kennel
cough vaccine as a separate inoculation.
FLEA CONTROL: All dogs must be on flea control. If we
find fleas on your dog, he or she will be bathed at your expense.
HEALTH: All dogs must be non-aggressive and not food
or toy protective. Owners will need to certify that their dog(s) have not
harmed or shown any aggressive or threatening behavior towards any person
or any other pet.
APPLICATION: All dogs must have a complete, up-to-date
application on file.
FEES: Due and payable in advance daily or weekly.
DAYS AND HOURS: Monday through Friday from 6:45am to
6:30pm. Your pet may not be dropped off before 6:45am and not be left here
after 6:30pm unless arrangements have been made in advance.
RESERVATIONS: Required. Preference will be given to
dogs attending full time. Cancellations with less than 24 hours notice
will be charged full fees.

Health and Temperament Agreement |
|
1. In agreement of being permitted to use the
services and facilities of The Muttley Crew, I/we, the undersigned Owner(s),
hereby release, waive, and discharge The Muttley Crew, its owners, staff,
and volunteers from all liability for any and all loss or damage, and any
claim or damages resulting there from, on account of injury, loss, damage,
infestation, or disease to my/our dog(s), even injury resulting in death,
whether caused by the negligence of The Muttley Crew, its owners, staff or
volunteers or otherwise while my/our dog(s) are under the of The Muttley
Crew.
2. I/we agree to indemnify The Muttley Crew,
its owner, staff, and volunteers for any loss, liability, damage, or cost
they may incur due to my/our presence or the presence of my/our dog(s) in
or upon The Muttley Crew's premises and while my/our dog(s) is/are under
the care of The Muttley Crew.
3. I/we hereby assume full responsibility for
any harm caused by my/our dog(s) while in our upon The Muttley Crew
premises and while my/our dog(s) is/are under the care of The Muttley
Crew. I/we further agree to indemnify The Muttley Crew, its owners, staff,
and volunteers for any loss, liability, damage, or costs they may incur
due to any harm caused by my/our dog(s).
4. I/we expressly agree that this release,
waiver, and indemnity agreement is intended to be as broad and inclusive
as permitted by the laws of the State of Oregon, and that if any portion
thereof is held invalid, it is agreed that the balance shall,
notwithstanding, continue in full legal force and effect.
5. I/we further understand and agree that in
admitting my/our dog(s) to The Muttley Crew, the owners of The Muttley
Crew have relied on my/our representation that my dog(s) is/are in good
health and has/have not harmed or shown aggression or threatening behavior
toward any person or any other pet.
6. I/we agree that should a court determine
that any provision waiving liability is deemed unenforceable, The Muttley
Crew liability shall be limited to the funds paid to it by me for taking
care of my/our pet.
7. I/we further understand and agree that any injury or
illness that develops with my/dog(s) will be treated as deemed best by The
Muttley Crew, and that I/we assume full financial responsibility for any
and all expenses involved, even if such expenses were later found to be
unnecessary. Furthermore, should my/our pet pass away during our absence
we direct that a veterinarian may be called to safe keep our pet until our
return.
8. I/we have read the attached Rules and Regulations and
agree to abide by them. I/we certify that I/we have read and understand
the Rules and Regulations set forth on the preceding page and I/we have
read and understand this Agreement and Release. I agree to accept all the
terms, conditions, and statements of this agreement.
